What is a connected care record and why does it matter?
A person who receives care has many interactions with people who get to know them well – people who are responsible for their care. Each of them will have information and knowledge relevant to their lives.
A medical professional will have specific insights into an individual’s health care, based on their training and expertise as well as medical history, interventions and medications.
A care worker may have built a personal relationship with the client, as well as having knowledge about their daily routines and unique preferences.
And, of course, family members will have unique memories and a special understanding of that person alongside expectations of what good care looks like for their loved one.
Each provides a valuable snapshot of that person’s life. But this information is too rarely shared and made accessible so that key decision makers can find it.
Client data is frequently fragmented or, worse still, lost or simply not recorded. To provide care to the standards expected by clients, their families and the regulators, a stronger system is required.
The creation of Connected Care Records is the answer to these issues. We need a unified ecosystem that captures all of that data and makes it accessible so it can inform every decision.
